If using occlusal technique and sensor with the intention to capture the root, trauma, & development and because it’s easier for the child to tolerate, is it correct to bill those images as D0220/D0230 or should they still be D0240?

Hi! No, you should not bill D0220 for an occlusal image. The intended use of a true PA is to capture the entire tooth crown to apex, and the surrounding bone. Occlusal radiographs are a different type of image that shows a broader area, not a focused periapical view. CDT coding is based on the type of radiograph taken, not the anatomy of what you are trying to see. If a true PA is taken, bill D0220 for the first PA. If an occlusal image is taken, bill the appropriate occlusal code D0240.
