What if after crown removal the tooth is deemed unrestorable and ext and implant are reccommeded?

The removal of the initial crown is not billable, it is considered an inclusive part of the new crown procedure. Most carriers will not cover a charge for the removal of an old crown to place a new one, however, if the carrier agrees to pay for the removal the most appropriate code would be D2999, this must be accompanied by a report or narrative.
