I don't this is allowed but I had to asked, because I can't find any guidelines regarding signatures for dental charting. Can a DDS chart and sign off a dental for another DDS (locum) that has left and is no longer with the dental office? ( this DDS did not chart a note at TOS). Where would be the best place to get information on dental guidelines, if there are any. A quick response would be very helpful. thanks.

What I believe you are asking is can a dentist document in the patients chart for another dentist? The answer is simply NO the dentist who provided the service must document . The only time another dentist may provide the documentation is if he/she was present at the time of procedure or examination and aided (more)

can insurance companies create their own billing for FMX? the CDT books says usually consist of 14-22 PA's and BW's but I'm being told by the dental biller that some insurances want the FMX code when less than 14 x rays are done. I would like clarification if this is ever appropriate to do. thanks.

Coding guidelines state 14-22 periapical films including bitewings must be present however, some insurance companies are paying out an FMX when a pano and bitewings are taken. The best course of action is to appeal the claim stating 14-22 films were not present at this visit.

Besides a dental hygienist who else can perform D0190 screening of a patient or D0191 assessment of a patient? and if these are billable services, if they are what are the billing requirements? there isn't much info on these codes in the Dental Coding Guide by Charles Blair DDS.

Now that ICD10 has been implemented. I have a question about who is allowed to assign diagnosis codes, specifically hygienists are they allowed to assign diagnosis codes?

The ruling from CMS is any provider whose scope of practice permits may assign a diagnosis code, (DDS, DMD, MD, Resident) it does not specifically state as to Hygiene. The common consensus is a hygienist may assign a diagnosis code however, it must be approved/reviewed by the attending/supervising provider.

this is what the provider documented "NP exam, pano, NV refer to pedo for space loss, decay, high anxiety" and charged for D0150 and D0330. is this sufficient documentation? I'm fairly new to dental and just wanted to clarify.

No this is not sufficient documentation for a D0150. A comprehensive exam must have documentation in the chart showing oral cancer screening, recording of patients dental and medical history, a general health assessment.
