You would use CPT code 41899 for all 32 teeth and send in a brief narrative of the different extractions and difference in pricing. You should note most medical carriers will not cover extractions unless they are full impaction, you should check your carrier guidelines before claim submission.
ICD-9-CM codes based on what was provided would be 523.9 and V41.6
