According to Delta Dental utilization review guidelines it is not appropriate to bill out an emergency palliative and a dental exam in the same visit.

Since you are performing a ridge augmentation where you are reducing the ridge instead of increasing the ridge a more appropriate code would be D7999. Submit with a brief explanation in the remarks area of the ADA 2012 claim form and use the appropriate ICD-10-CM code as explanation.

No there is no cross code for this procedure as it is not covered by medical carriers.

The most appropriate code would be D2999 and submit a brief narrative in the remarks box as to what is being done and why.

Yes, you may bill for the N2O however you must have a brief explanation accompany the claim.

You can, however, depending on the carrier this may not be covered if D5130 is not delivered on the same day.

You do not need an affiliate ID number unless someone has referred you to the Association, the affiliate ID is to give that person credit. Coupon codes are for military discounts and are not needed unless you are military. You do not need to fill anything in these areas unless you are instructed to do (more)
