    UHC/ PH denying D2954
    I keep getting denials for a D2954 when the patient has an existing crown and needed a rct and the Dr did a post and core after . What is the best way to file that?

    Hi! I would recommend sending a pre and post-operative x-ray of the tooth, the clinical notes showing the Doctor's diagnosis, and a narrative from the provider stating why they determined that the best care for the patient was a D2954.

    why is the insurance company requiring surfaces for a D6240

    Unfortunately, I can't exactly explain why a payor would request a tooth surface, but it may be a requirement of the plan for the code. This is probably given to be sure that duplicates are not billed. Also, they are typically subject to a 5–10-year limitation. There can be quite a few limitations to these (more)

    CAN D9222 BE BILLED TWICE ON ONE DATE OF SERVICE?
    PT CAME IN THIS MORNING HAD TOOTH #8 EXTRACTED WITH D9222 AND D9223 PT RETURNED THIS AFTERNOON AND HAD TOOTH #9 EXTRACTED WITH D9222 AND D9223 CAN I SUBMIT 2 SEPARATE CLAIMS TO INSURANCE WITH EACH HAVING D9222 LISTED?

    It is very likely that the insurance is going to deny the second D9222 and D9223. I would submit medical records indicating the need for general anesthesia the second time (twice in a day).     What would the code be for toothbrush deplaquing

    There is not a separate code for toothbrush deplaquing. This service would be included in the prophy code, either D1110 (adult) or D1120 (child).

    How to charge for downcoded D4260
    D4260 may be downcoded to D4261 due to periodontal charting . Isf full quad of surgery is done, is the patient responsible for the difference up to the full quad fee of the limited quad fee?

    You're stating that the insurance downcoded the procedure based upon the evidence of the perio chart?     Can I charge D0801 when I am also charging later date D2740?
    We use digital scan rather than conventional PVS and I'd like to know if I can now charge for 3-D dental surface scan -direct when scanning for a porcelain/ceramic crown fabrication? I will be posting D2740 on delivery date of crown.

    The code for the sectioning of the bridge is D9120 and depending on the type of extraction performed it is either D7140 or D7210
