I would advise my coders to assign D7300 because it is the corresponding code to the placement codes and was meant to remove a temporary device. CDT 9987 reports a cancelled appointment. The CDT description does not indicate whether or not the procedure has been cancelled by the provider or patient. If you are not using the code to charge a fee for cancelled appointments, then it would be appropriate. Of course the notes should show why the appointment was cancelled. (more)

You would bill D0145 until the child turns 3 years of age, this code is specific to oral evaluations for under 3. Then bill D0150 at 3 years of age and D0120 every visit after.
