CDT 9987 reports a cancelled appointment. The CDT description does not indicate whether or not the procedure has been cancelled by the provider or patient. You would bill D0145 until the child turns 3 years of age, this code is specific to oral evaluations for under 3. Then bill D0150 at 3 years of age and D0120 every visit after.
