If you had to re-cement or re-bond the space maintainer the code would be D1550. If you had to place a new space maintainer the code would be D1999 accompanied by a brief narrative as to why the space maintainer had to be replaced.

If this claim was denied due to timely filing an appeal is not going to change the outcome. You need to read your contractual obligation on claim submissions with the carrier and proceed according to their guidelines. 1. There is no code for taking a patients picture for identification purposes nor is it a billable charge. 2. depends on the type of x-ray taken and if it was supervision and interpretation only.
