Did you submit the medical records for the necessity of the procedure? Be honest and open in your documentation - send medical records indicating the necessity of the procedure and a diagnosis to support what was done. We are seeing children under 3 for sometimes 3 appts and then charging out a D0150 . Wondering if we can chg a D0120 for a visit and then a D0150

You would bill D0145 until the child turns 3 years of age, this code is specific to oral evaluations for under 3. Then bill D0150 at 3 years of age and D0120 every visit after.

I was charged code D4266 by my dentist and my insurance changed it to code D7956 and they covered it. I had to pay out my pocket for code D4266. Should I ask for a refund for the D4266 charge?

Except that D7550 is removal of sloughed off bone due to infection and blood supply. The RVUs assigned to this for payment do not constitute removal of a simple bone spur.

Should a Dentist charge a separate lab fee for this type of crown

Agree with prior post, dentist do not usually charge a lab fee. BE careful charging more than the allowed amount on contracted plans and covered services; that is a compliance no no.
