Depending on the documentation provided and if the patient is new or established within the practice the correct CPT code(s) for D0140 would be 99201-99202 for a new patient or 99212-99213 for an established patient.

You will find these codes by going to your index and looking up the main term Evaluation and Management, from there you would go to the subterm office and other outpatient this gives you a code range of 99201-99215

The correct code for D6050 depending on the insurance carrier is 21248 partial or 21249 complete.
Depending on the carrier some feel partial means the 1-3 teeth and complete means 4 or more teeth, while other carriers deem partial to mean the surgical phase and complete to mean the surgical and prosthetic phase. You will need to check with your carrier for accuracy.

You will find these codes by going to your CPT index located in the back of the book. You would look up the main term Reconstruction, then the subterm mandible with implant or maxilla with implant depending on which part of the mouth you are working on this will give you a code range of 21244-21249

The correct CPT code for D7260 is 30580

You will find this code by going to the index of your CPT book and looking up the main term Repair, then the term nose, and the subterm fistula this will give you code range 30580-30600

Once you find your code range you will flip to the section of codes and see which one best describes your procedure.
