Yes, you may make a correction on the claim form in the remarks area stating "Claim Correction" and refile with code D1352.

D2950 and D2952 are not to be billed in conjunction with one another, you either get D2950 (core build-up including pins) or you get D2952 (custom fabrication).

Usually this would be considered an inclusive part of the original procedure and is not billable. However, you may try billing D9999 and include a brief narrative for consideration.

If you are submitting the claim to dental you would use D9248 if it is being administered intramuscular or D9241 if it is being administered by IV. If you are removing gingiva for the purpose of giving more tooth structure for a crown the most appropriate answer is D4249 crown lengthening as this would not be considered a gingivectomy. This is really a carrier driven question, some carriers will allow the exam to be performed on the same day as the debridement, however most carriers will not.
