If a Dr. writes the description (example: pupal necrosis secondary to decay)of a diagnosis code in the patients chart note but does not provide a the code can a certified coder assign the diagnosis code on the claim form?

Yes, as a coder you are responsible for code selection and assignment based on provider documentation.

1. Can a bill be submitted to a dental insurance for co-pay payment after claim was paid by medical insurance. 2. If so, naturally we would need to change the billing code from medical to dental. COrrect. Ex. TMJ appliance 21089 medical to D7880 . Thank you.

Yes, you may bill dental as a secondary carrier to the medical plan payment. You will need to change the code on the dental claim form and submit the EOB from medical along with stating the code description of CPT code in box 35 of the ADA claim form

Can we bill for code D0470 along with code D7880 for a TMJ Appliance. Also if we are billing D09940 for an occlusal splint

You may bill D0470 along with D7880 as long as the casts are for diagnostic purposes and not being used to create the device. D7880 includes splints and therefore D9940 would be considered inclusive of D7880.

What is the appropriate code for Space Maintainer repair.

If you had to re-cement or re-bond the space maintainer the code would be D1550. If you had to place a new space maintainer the code would be D1999 accompanied by a brief narrative as to why the space maintainer had to be replaced.

I have a person that is a patient but has not been seen in our office for over 5 years. Do I consider him a new patient and if so what is the time frame for that.

Any patient who has not been seen by any provider in the same practice in over 3 years is considered a new patient.

I received a denied claim due to the DOS being past the time limit allowable (6 months). Does anyone have a good narrative to appeal this claim denial?

If this claim was denied due to timely filing an appeal is not going to change the outcome. You need to read your contractual obligation on claim submissions with the carrier and proceed according to their guidelines.
