    There is no code specific to just reattaching the locator abutment. I would say that you should bill D6199 and include documentation reporting what was completed.

    How to charge for downcoded D4260
    D4260 may be downcoded to D4261 due to periodontal charting . Isf full quad of surgery is done, is the patient responsible for the difference up to the full quad fee of the limited quad fee?

    Are toodh numbers needed when billing D7210 for 2 different teeth on Hospital Facility UB claim?

    Since they are a requirement by some insurance companies on the CMS-1500 form I would say that it is also a good idea to enter them on the UB-04 form, but I cannot find documentation that states that it's a requirement. You might want to check with the insurance carrier to be sure.

    Most of the descriptors I am seeing for this code state this is not a definitive restoration. Therefore, if this code is used should the patient be returning for a permanent restoration?

    I've never submitted to medical and don't have claim forms

    In order to bill medical insurance, you would need to contract/use an online company to bill medically or purchase a stack of CMS-1500 forms and complete the forms in your office to send to the insurance company.     I would not send a narrative for this procedure, but the very detailed clinical documentation stating the necessity of the procedure. Unless the patient has implant coverage with their insurance carrier, in most cases the procedure is likely to be denied.
