You do not need an affiliate ID number unless someone has referred you to the Association, the affiliate ID is to give that person credit. Coupon codes are for military discounts and are not needed unless you are military. You do not need to fill anything in these areas unless you are instructed to do (more)

The AAPC and the BC Advantage are currently running ICD-10 training courses. The ADCA will not have an ICD-10 training course until January 2016.

They are looking for qualifying circumstance modifier found in the CPT coding manual... D7321 Alveoloplasty not in conjunction with extractions, and D7285 incisional biopsy of oral tissue are considered separate procedures.
